Notice of Infringement – Claim

- Contact information of the complaining party;
- A statement of good faith belief of unauthorized use;
- A statement of accuracy of the information provided.

Counter Notification – Restoration of Material
If you receive a takedown notice, you can send a counter notification to have the material restored. This must include:
- Your signature;
- Description of the removed material and original location;
- A statement under penalty of perjury that the material was wrongly removed;
- Your contact information and consent to jurisdiction.
